The Medical Records Department prepares and maintains patient records. Primarily an information resource for physicians at the medical center, this department also provides information to patients, including medical reports, medical histories and physicals, operative reports, consultations and discharge summaries.

Somerset Medical Center is committed to protecting the confidentiality of health care information. Upon proper authorization, release of information can be requested for a minimal charge. To request your records, complete an Authorization for Release of Medical Information form (click here for the Spanish version of the form). All applicable sections on both pages of this form must be completed. Incomplete forms cannot be processed and will be returned for additional information. Return completed authorization forms to:
